What the forms collect

The general contact form asks for your name, a phone number, an email address, and, optionally, your company, the kind of program you are asking about, whether it is DOT or non-DOT, whether you need collections on site, roughly how many employees are covered, and anything you want to tell us before a call. The short confidential form, used on the return-to-duty and student testing pages, asks only for a name, one way to reach you, and an optional message, and it does not record which page you were on. Nothing else is requested and nothing is required beyond what each form marks as required. Please do not send test results, medical information, or Social Security numbers through a website form.

How a message from this site travels

When you press the button, the form is sent over an encrypted connection to the site's hosting account at Cloudflare, which PSI controls. Nothing is sent from your own email account or through your employer's mail system. Before the message is accepted, a bot check by Cloudflare Turnstile confirms a person sent it; that check may set a short-lived cookie for that purpose only.

Each message is stored in a database in that hosting account for up to twelve months, so that nothing is lost if an email goes astray, and is then deleted. A copy is emailed to the office from PSI's own mail account, the same one the office uses every day. From there the message is handled like any other correspondence with the office. Messages are used only to answer you; they are not used for marketing, and they are not sold or shared. If you would rather not use a website form for a personal question, call 732-869-2900 instead.

Sending a message through this site never asks you to sign or agree to a consent, release, waiver, or indemnity. Under 49 CFR 40.355(a), no service agent may require one for any part of the testing process.

What this site does in your browser

The site does not set advertising cookies, does not sell or share personal information, and does not build profiles of visitors. Our hosting provider keeps ordinary server logs, which include the address of the page requested and technical details such as a browser type, and those logs are used to keep the site running and secure.

Testing records are a separate matter

Drug and alcohol testing records are governed by federal regulation, separately from this notice. Under 49 CFR 40.321, PSI does not release an individual test result or the medical information behind it to any third party without the employee's specific written consent. Sections 40.323 through 40.333 set out the defined situations where release is required or permitted anyway: to the employer, to DOT agency representatives on request, in specified legal proceedings, to a person the employee names in a specific written consent, and to the employee, who may ask for a copy of their own records under 49 CFR 40.329 and receive them within ten business days. Records are held, and released, only on those terms.

Testing records belong to the employer whose program they document. Under 49 CFR 40.349(f), PSI transfers all records to the employer, or to the service agent the employer names next, on the employer's request, without employee consent and for no more than a reasonable administrative cost. Under 49 CFR 40.355(n), no service agent may delay a testing document because of a payment dispute.

Student testing records

Where PSI supports a school district's student testing program, student records are education records under the Family Educational Rights and Privacy Act, 20 U.S.C. 1232g. PSI handles them only under the district's direction and only as the board-adopted policy provides.
